Questions about official Polk County criminal records usually belong with the government office that created or maintains the file. The contacts below route case documents to the circuit clerk, Sheriff reports and bookings to Sheriff's Records, and statewide criminal-history requests to Wisconsin Department of Justice. This private resource does not receive or process agency requests.

Need an Official Record?

Send each Polk County request to the office responsible for that record:

  • Circuit case files: contact the Polk County Clerk of Circuit Court, or review the criminal court case record routes
  • Sheriff case reports: use Polk County Sheriff's Case Report Request Form or contact Sheriff's Records
  • Bookings and jail records: contact Polk County Sheriff's Records or the Polk County Jail
  • Wisconsin criminal history: request the appropriate result from Wisconsin DOJ, Division of Law Enforcement Services, Crime Information Bureau
  • City police reports: contact the police department that created the report
  • Expungement: ask the court that sentenced the case whether expungement was ordered under Wisconsin law

Polk County Clerk of Circuit Court is in Suite 300 at the same address. The main line is 715-485-9299, the criminal division is 715-485-9227, and fax is 715-500-6525. The clerk is open Monday through Friday, 8:30 a.m. to 4:25 p.m.
